Enrollment Application Status Inquiry (EASI)

The EASI application allows you to check the status of your provider enrollment application in real-time.

Upon receipt of your application, WPS sent you an email acknowledging your request and providing you with an Application ID.

To view the current status of your application, please enter the email address to which the notification was sent as well as the Application ID.

This web site is not hosted on a Centers for Medicare & Medicaid Services (CMS) web server.
